I accept private medical insurance, both in-network and out-of-network. For out-of-network, I can provide you with a superbill which you can submit to your health insurance for reimbursement.
Self-pay rates:

Follow-up visit:
$100 for 1/2 hour
$150 for 1 hour

ESA letters, FMLA, long-term and short-term disability forms: $100 for each form
